Apply for an aged care assessment

Support to apply for an aged care assessment online through myagedcare.gov.au. 


Aged care services are for people aged 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people).

Centrelink help

  • Centrelink Carers Allowance applications (form SA426 or SA332a)
  • Centrelink Carer Payment – Medical Report  (form SA427)
