This is charged pro rata. For example, the fee for 10 minutes of travel is $41.24. This is equal to $247.44/ hour.

The NDIS will cover the costs in full.
An invoice will be issued after the appointment for payment. The applicable amounts can then be claimed back from the NDIS after payment.
"You can also [self] manage parts of your plan, but have a plan manager or the NDIA to help you manage the rest. You can ask us [the NDIA] to change how you manage your plan at any time".
"We may include NDIS supports in your plan that you can use to help you learn how to self-manage.
